With a commitment to advocacy and community engagement, the AAA7 supports safe and independent living through long-term care home and community-based programs and resources.
Services and Programs
Resource Center Specialists provide callers with up-to-date, reliable information and helpful assistance in determining options and choices that might be available for them in the community. Through completion of the Long-Term Services and Supports Questionnaire (LTSSQ), staff will identify needs and provide information regarding available agency and community services and programs.

Helping You Age Better at Home
Ohio's Assisted Living Waiver Program pays the costs of care in an Assisted Living facility for Medicaid recipients, allowing the consumer to use his or her resources to cover "room and board" expenses.

Hospital2Home
The Hospital2Home program is a voucher program for those eligible who are being discharged from hospital to home. Vouchers allow for the purchase of short-term services such as personal care and home-delivered meals to assist individuals returning to their homes after a brief hospital stay.
PASSPORT
PASSPORT is designed to keep older adults at home or to help them return home after a nursing facility placement. Consumers in the PASSPORT Program have the choice of either the traditional option where services are provided through agency or non-agency professional caregivers, or participant direction option, where services are provided by individual providers such as friends, neighbors or some relatives.

Service Coordination
Service Coordination places a qualified individual in your community to help elderly and disabled residents access services and programs they may need. A Service Coordinator is on site to offer assistance and information, make referrals, promote healthy lifestyles and wellness, encourage common interests and activities, and improve quality of life.
Adult Day Services
Adult Day Service is a structured program in a protective setting to help meet the needs of a person who has functional or cognitive impairments. This service is available through several funding sources, including private-pay.
Congregate Meals
Congregate Meals is a service that provides a nutritious, safe and appealing meal in a congregate (group) setting which promotes socialization. Each county in our district has at least one congregate meal site.
Home-Delivered Meals
Home-Delivered Meals is a service that provides a safe and nutritious meal in the home setting in order to sustain or improve the person's health. A home-delivered meal is provided to anyone age 60 or over (and their spouses, regardless of age) at no cost - although a donation is encouraged.
Homemaking
Homemaker Services help the person maintain a clean, safe and healthy environment. The activities may include dusting furniture, sweeping, vacuuming, mopping floors, removing trash, washing dishes, changing bed linens, doing laundry, and other household tasks.
Legal Services
Legal Services provide legal advice, counseling, and representation by an attorney or other person acting under the supervision of an attorney. Representatives from Ohio State Legal Services or Legal Aid of Cincinnati make appointments in each county.
Transportation
Transportation is a service that provides transportation for medical appointments, particpation in a congregate meal site or senior center, and other errands. This service is available through several funding sources, including private-pay.
Personal Care
Personal Care Services assist a person with personal hygiene, dressing, bathing, toileting, eating, and transferring in and out of bed or chairs, in order to maintain or improve their independence. Homemaker activities may also be included in the assistance.

Eligible seniors in our 10-county region (Adams, Brown, Gallia, Highland, Jackson, Lawrence, Pike, Ross, Scioto and Vinton) may qualify for critical home repairs and accessibility upgrades through AAA7’s Home Repair Program. The program is funded by the Ohio Department of Development Housing Trust Fund Housing Assistance Grant Program and the State of Ohio Senior Community Services Program.

Ohio’s HOME Choice program transitions eligible Ohioans from institutional settings to home and community-based settings, where they receive services and supports at home and in their communities. Individuals and family members/guardian will work with the long-term care facility and HOME Choice transition staff to apply, discuss options and coordinate services and supports.

Ombudsman Program Overview
The Ombudsman provides a voice for consumers of long-term care. These consumers include residents of nursing homes, residential care facilities, and adult care homes as well as recipients of “in-home” services.

Volunteer Ombudsman Program
The Ombudsmen Program recruits and trains Associates who visit assigned nursing homes on a weekly basis. Ombudsman Associates also help to educate residents about their rights, thus empowering them to become more vocal in resolving their own complaints and concerns.

The AAA7 Caregiver Support Program assists caregivers in finding resources and services to help decrease the physical and emotional stress in their caregiving role in order to take better care of themselves and their loved ones. Services also assist grandparents caring for grandchildren.

Through partnerships with local VA Medical Centers, AAA7 helps eligible Veterans self-direct the care they need to live safely and independently at home. To be eligible for the program, a veteran must be enrolled with the VAMC, have a VA primary care provider, and meet the eligibility criteria for home and community-based services as determined by the VAMC.
Interested Veterans should talk with their primary care team at the Chillicothe, Cincinnati or Huntington VAMC.
Bingocize
“Bingocize®” is a free evidence-based health promotion program that combines the game of Bingo with falls prevention exercise. Trained leaders facilitate a 10-week workshop that includes exercises to target range of motion, balance, muscle strengthening and endurance

Matter of Balance Falls Prevention
A Matter of Balance is an award winning falls prevention program designed to reduce the fear of falling and increase the activity levels of older adults who have concerns about falls.
The program uses trained volunteer coaches to teach eight two-hour sessions.

Tai Chi for Arthritis
“Tai Chi for Arthritis and Fall Prevention” is a free program that helps participants improve overall health and well-being, exercise the whole body, and reduce stress while calming the mind. Through this evidence-based class, the AAA7 helps individuals age 60 and over improve movement, balance, strength, flexibility, immunity and relaxation; decrease pain and falls; and increase socialization and sustainability.

Low Income Subsidy Education & Enrollment
Anyone who has Medicare can get Medicare prescription drug coverage. Some individuals with limited income and resources may also be able to receive “Extra Help” with the costs such as monthly premiums, annual deductibles, and prescription co-payments related to a Medicare prescription drug plan.

Medicare Part D Education & Enrollment
People with Medicare may enroll in Part D coverage through either a stand-alone plan or a Medicare Advantage plan. People on Medicare who choose not to enroll and have no other drug coverage may pay a penalty for late enrollment.
Ohio Senior Health Insurance Information Program
The Ohio Senior Health Insurance Information Program (OSHIIP), founded in 1992, is a state and federally funded program that educates, counsels, and advocates for Medicare beneficiaries. OSHIIP assists Ohio Medicare beneficiaries and their family members through a toll-free hotline at 1-800-686-1578, the Department web site, a speakers’ bureau, educational publications, and a network of approximately 1,200 statewide trained and certified volunteers.

SHIP is a voluntary service in Ross County and Scioto County that provides a free home safety inspection to identify features in your home that may make your residence susceptible to burglaries, fires, or injuries due to falls. Persons eligible must be age 60 or over and reside in either Ross or Scioto County.
For additional information about the SHIP program, contact: for Scioto County, the AAA7 Resource Center at 1-800-582-7277; and for Ross County, contact the Chillicothe Fire Inspector at (740) 773-2212.
To be eligible for $50 to use with authorized farmers in the program, individuals must live in the ten counties included in the AAA7’s district which include Adams, Brown Gallia, Highland, Jackson, Lawrence, Pike, Ross, Scioto and Vinton. Individuals must be age 60 or over at the time of application and households must earn less than 185% of the poverty level.
To support individuals and their caregivers, AAA7 offers a range of services including in-home respite care, adult day services (available in Gallia and Scioto counties), and education and support through the Alzheimer’s Association. These programs provide relief for caregivers, social engagement for individuals with dementia, and valuable resources for families and professionals.

HEAP and PIPP offer energy assistance to eligible Ohioans based on household income, with most benefits applied directly to your utility bill. You may also qualify for weatherization services to improve your home’s energy efficiency.
