Medicaid Waiver Services

PASSPORT Program:​​​

A Medicaid waiver alternative to nursing home that supports older adults with services that include but are not limited to the following:​​​​

  • Personal Emergency Response System

  • Non-Medical Transportation

  • Home Medical Equipment & Supplies​

  • Personal Care

  • Homemaker Services

  • Home-Delivered Meals

  • Adult Day Care

How to Become a PASSPORT Provider:

​​​

The Area Agency on Aging District 7, Inc. (AAA7) Provider Relations Department works with current service providers and potential service providers to provide needed services for eligible older adults living in Adams, Brown, Gallia, Highland, Jackson, Lawrence, Pike, Ross, Scioto and Vinton Counties. PASSPORT is Ohio’s alternative to nursing home care.  PASSPORT helps low-income, disabled older adults remain safe and independent in their home. Services offered on PASSPORT include Adult Day, personal care, homemaker, Consumer Directed (Choices Home Care Attendant), home maintenance and chore, personal emergency response system, home medical equipment and supplies, home modifications, home delivered meals, community transition services, non-medical transportation, structured family caregiving and more.

CERTIFICATION PROCESS FOR THOSE SEEKING CERTIFICATION FOR THE FIRST TIME:​​​

​

Providers of services to Ohio Department of Aging (AGE, formally known as Ohio Department of Aging, ODA) may fall into different categories: agency-based service providers, non-agency-based services providers, participant-directed (Choices Home Care Attendant Services), or assisted living providers. 

​

Agency and assisted living provider applicants must pay a non-refundable fee and include supporting documentation with their application. The application fee is equal to the amount established by CMS and includes an annual adjustment for inflation in accordance with 42 U.S.C. 1395cc. 

​

All provider applicants must meet requirements outlined in Ohio Administrative Code. 

Providers may apply online with AGE at Certification Information | Department of Aging.

  • Agency Providers: are a legally organized entity which employs staff to perform the type of services the agency is seeking to be certified. 

  • Non-Agency Providers: are a legally organized entity which is owned and controlled by one person and does not employee staff. 

  • Participant Directed, Choices Home Care Attendant Services: The person the individual (consumer) hires becomes certified through the Ohio Department of Aging (AGE) instead of getting hired as an employee through a certified personal care service agency. The individual is the employer, and the provider is the employee.  

  • Assisted Living Providers: An Assisted Living Waiver provider is a residential care facility licensed with the Ohio Department of Health. Assisted living combines a home-like setting with personal support services. Assisted living facilities typically provide more intensive care than is available through home care services. They also give older adults an alternative to nursing facility care that is both less expensive and less restrictive.

Providers are subject to the following rules and any rules within them as applicable for their service specification:​​​

  • The provider should subscribe to receive email updates on ODA's rules through here 

​

  • When a completed application packet has been submitted. AGE will contact The Area Agency on Aging District 7, Inc (AAA7).

​

  • AAA7 will contact the agency to conduct a pre-certification review (either in person or desk review) to determine if the provider meets the AGE provider certification requirements and the service specifications they applied for.

​

  • After the review, you will receive a Pre-Certification Summary letter or email.

  • The recommendation to become a certified PASSPORT provider will be sent to AGE. AGE will make the final determination. 

  • If AGE approves the certification, AAA7 will be notified and will take the next steps necessary to finalize your certification.  

  • If AGE denies certification, the provider will receive notification from AGE, and will be ineligible to reapply for certification for one year

PROVIDER REVALIDATIONS:​​

​​

Per  Rule 5160-1-17.4 - Ohio Administrative Code | Ohio Laws, revalidation is the process that a provider is required to follow to renew and revalidate its provider agreement with the Ohio Department of Medicaid (ODM). Provider agreements must be revalidated no later than five years from the effective date of the original or the last revalidated provider agreement, whichever is applicable. Failure to revalidate can result in the ODM terminating the provider agreement. Revalidation is completed through the Provide Network Management (PNM) system.

Providers may call the Integrated Help Desk at 1-800-686-1516 and follow the prompts to Provider Enrollment with any questions about revalidation or recredentialing.

Per Rule 173-39-03.4   If you are already certified with AAA7 and seeking to expand services OR if you are already certified with AGE in another region and seeking to expand to AAA7’s region, please complete the expansion request form:

DIRECT DATA ENTRY (DDE) EXISTING PROVIDERS:​

​

The Direct Data Entry (DDE) program is a web-based billing program for agency, non-agency and assisted living providers.  Consumer directed (Choices Home Care Attendant providers us PPL and will receive information from that site once authorized to provide services)

​

NEW PROVIDERS:

  • To obtain your username and password you must be authorized for services on a service plan and then must complete the Provider DDE user request form 

​

NEXT GEN MYCARE:

​

Next Gen MyCare will be coming to AAA7 in 2026 with a phased in approach from June-September 2026. We encourage all agency providers to consider enrolling with this program to ensure members continue to have access to choice for their care. 


To become a Next Generation MyCare program provider, please complete the following: 
   •    Enroll with the Ohio Department of Medicaid (ODM) by visiting the Medicaid Provider Portal and complete the online application (credentialing, if required, will occur during application processing). 
   •    Contract with the Next Generation MyCare plans by contacting each of the plans you wish to contract with.

Next Gen MyCare plans are not required to contract with every provider. If a provider does not have a contract with a Next Generation MyCare Plan, they may still be able to set up a noncontracted reimbursement agreement (single case agreement).

Regional Long-Term Care Ombudsman Program

Older Americans Act Programs

Potential providers seeking contracts to deliver services funded under the Older Americans Act (OAA) may only apply during an open Request for Proposal (RFP) process. The RFP is the formal and competitive method through which the Area Agency on Aging District 7, Inc. identifies and selects providers.

​

Applications are not accepted on a rolling basis; instead, they must be submitted within the designated timeframe outlined in the published RFP. This ensures all applicants have an equal opportunity to compete for funding, promotes fairness and transparency, and allows the Agency to evaluate providers based on consistent criteria.

​

Interested providers should monitor Agency communications and public notices for announcements regarding any upcoming RFPs. Only proposals received in response to an active RFP, and submitted according to the stated requirements and deadlines, will be considered for contract awards. The next RFP will be in the spring of 2026 for the next Area Plan period for 2027-2030.

Home Repair Program Contractors

The Area Agency on Aging District 7, Inc. offers a home repair program to eligible older adults in our ten-county region. The program is funded by the Ohio Department of Development Housing Trust Fund Housing Assistance Grant Program and the State of Ohio Senior Community Services Program. Partnership is also given from the United States Department of Agriculture Section 504 Program.

​

Services offered include repair or replacement of heating systems; plumbing systems; hot water tanks; water taps and lines; hazardous electrical systems; severely deteriorated roofs, walls, floors and doors; ramps for handicapped-accessibility; and kitchen and bathroom modifications for handicapped-accessibility. Some restrictions apply for mobile homes.

​

AAA7 is required to have approved contractors to perform the home repair/home modification work. If you are interested in becoming a Home Repair Program contractor, please contact Kristy Bowman at 1-800-582-7277, extension 22250.
