From service coordination to assisted living options, we're here to help you navigate your choices and improve your quality of life in the comfort of your home.

Assisted Living
Ohio's Assisted Living Waiver Program pays the costs of care in an Assisted Living facility for Medicaid recipients, allowing the individuals to use his or her resources to cover "room and board" expenses.
Assisted living residences vary considerably, but most provide meals, housekeeping, laundry, transportation, and social activities. They also offer personal care, such as assistance with eating, bathing, grooming and personal hygiene. Some nursing care is also provided, including medication administration and dressing changes.
Participants must be financially eligible for Medicaid and determined by the Area Agency on Aging District 7, Inc. to need a "nursing facility level of care," the same requirements as for the PASSPORT program. Eligible consumers can select from any participating licensed assisted living provider.

Hospital2Home Program
The Hospital2Home program is a voucher program for those eligible who are being discharged from hospital to home. Vouchers allow for the purchase of short-term services such as personal care and home-delivered meals to assist individuals returning to their homes after a brief hospital stay. The program will also offer short-term telephonic assistance to help with redeeming the vouchers if necessary and assist in the exploration of ongoing long-term services and supports.

Eligibility Requirements

Age 60 or older and a recent hospital stay.

Must Reside in the AAA7’s 10-County Service Area.

Do not currently have a Medicaid card or currently enrolled in a Medicaid waiver program such as PASSPORT.

To receive personal care services, must need assistance with at least two ADLs (Activities of Daily Living) such as bathing, dressing, toilet use, transferring, eating, walking.

To receive the home-delivered meal service, must be dependent with the IADL (Instrumental Activity of Daily Living) of meal preparation and not have someone available to prepare the meal.

Must be living in a home-based setting, not a facility

PASSPORT
PASSPORT is designed to keep older adults at home or to help them return home after a nursing facility placement.
PASSPORT is a service of the Area Agency on Aging District 7, Inc. and is made possible by funds through the Ohio Department of Aging (ODA). Funds are also through the Ohio Department of Medicaid.

individuals in the PASSPORT Program have the choice of either the traditional option where services are provided through agency or non-agency professional caregivers, or the self-directed option, where services are provided by individual providers such as friends, neighbors or some relatives. In the self-directed option, the individual is the "employer of record" for individual providers and is responsible for hiring, firing, training, and completing all necessary tax forms and payroll duties for these workers. A Fiscal Management Service is provided to assist the individual with the legal and financial aspects of the program. Staff at the AAA7 help individuals understand their options through the PASSPORT Program so that they can make the best decision that will support their health and independence.

Service Coordination
What is Service Coordination?
Service Coordination places a qualified individual in your community to help elderly and disabled residents access services and programs they may need. A Service Coordinator is on site to offer assistance and information, make referrals, promote healthy lifestyles and wellness, encourage common interests and activities, and improve quality of life.
