Interested providers must make application with the Ohio Department of Job and Family Services (ODJFS) through Ohio's Medicaid Information Technology System (MITS).

 

Initial PASSPORT Provider Certification

The Quality Assurance Department is available 8:00 a.m. through 4:30 p.m., Monday through Friday at 1-800-582-7277. Also, the Area Agency on Aging, District 7 Quarterly Newsletter is mailed directly to all certified PASSPORT providers. 

 

All Medicaid providers must register to access and update their demographic information. Instructions for provider registration can be found at http://www.jfs.ohio.gov/ohp/provider.stm. For assistance with MITS, contact the ODJFS Provider Call center at 1-800-686-1516.
