CHOICES Overview
General Information
Once a person has been approved for the PASSPORT program and has services in place, then the consumer may opt to be referred to the CHOICES program, a consumer directed Medicaid waiver program that provides home and community-based services and supports to older Ohioans. Choices Providers can be agency or non-agency professional caregivers or individual providers such as friends, neighbors or some relatives for a Choices consumer.
CHOICES is a service of the Area Agency on Aging District 7, Inc., and is made possible by funds through the Ohio Department of Aging (ODA).
Participant Information
Persons served will:
- Be age 60 or over
- Meet income guidelines
- Need assistance with daily activities
- have Physician approval services
- Be a current PASSPORT consumer referred to the Choices program.
Eligible Provider Information
You must be a certified PASSPORT service provider or a certified Individual Provider in order to provide services to CHOICES consumers. The initial enrollment process is somewhat lengthy and will take several months to complete.
The application materials for those wishing to be a certified individual provider for a Choices consumer can be obtained from that Choices consumer.
