Assisted Living Fast Facts
Enrollment Process:
Consumers are referred to Screening;
Must be assessed and found eligible before being put
on the waiting list;
Must sign a 2399;
Slot and room must be available (limited slots and
limited rooms)
Eligibility:
Must meet ILOC and have a need for unscheduled
services, i.e. toileting, falls, etc.;
Must be a NF resident or enrolled in a Waiver
program, i.e. PP, Ohio Homecare Waiver;
Providers:
Consumer must be willing to live in one of the
certified homes;
Consumers can choose any certified provider in the
state;
If a consumer is in PSA 7 and wants to move to
another PSA he/she must be enrolled in PSA 7 then
transferred.
Finances:
Consumer is allowed to keep $50 per month for
personal needs;
The rest of consumer’s funds go to the provider to
pay a room and board fee of $573 and a liability
fee;
The provider bills the PSA for the AL services based
on a tiered level (Tier 1, 2, or 3);
The AL CM assigns the tier based on the consumer’s
care needs (Tier 1 is lowest care needs and Tier 3
is the highest);
Estate Recovery applies to the AL Waiver; if a
consumer owns a home it will eventually need to be
put up for sale same as if they were entering a NF.
Services provided:
Assist with personal care, homemaking, medication
administration, 3 meals per day, non-medical
scheduled transportation, and nursing services.
Some AL homes have secure units and some do not. If
a consumer requires 24-hour supervision due to
cognitive impairments, he/she may not be appropriate
for AL. AL staff will assist with making the
determination of appropriateness.
