Care Coordination Case Management
WHAT IS SCSBG/CARE
COORDINATION? The
Senior Community Services Block Grant/Care
Coordination program is intended to assist seniors
who need minimal service to continue their
independent living arrangements at home or who are
financially or medically ineligible for other home
care programs.
WHO COORDINATES AND ADMINISTERS
THE PROGRAM? Care
Coordination is funded by a Senior Community
Services Block Grant allocated to the Area Agency on
Aging District 7, Inc., from the Ohio Department of
Aging.
WHO MAY BE
ELIGIBLE FOR SCSBG/CARE COORDINATION?
Individuals who are 60 years of
age or older, reside in the ten county service area,
and meet financial and need requirements.
Eligibility is determined after an assessment
completed by an assessor from the Area Agency on
Aging.
CARE COORDINATION SERVICES:
Home-Delivered Meals,
Personal Care, Homemaker, Alzheimer's Respite,
Medical Transportation, Case Management
WHAT IS THE COST?
Most eligible seniors who are
enrolled will find they have no charge for the
provided services. Those above the established
financial guidelines--based on the federal poverty
guidelines--may share in the costs of the provided
services. The amount will be determined at the
initial assessment and agreed upon before services
begin.
HOW TO GET STARTED?
Anyone can refer an older adult
for services by calling 1-800-582-7277. After the
referral information is taken, a face-to-face
assessment with the prospective consumer will be
scheduled for a later date. If an individual is
determined to be ineligible for Care Coordination,
other referrals or services could be discussed with
the individual.
Most of the
services listed are available in all counties.
However, there are some exceptions. Check with the
Area Agency on Aging for specifics.
