What is SCSBG/Care
Coordination?
It is a program intended
to assist seniors who need minimal service to continue
their independent living arrangements at home. The
program is intended to assist those seniors who are
financially or medically ineligible for other home
care programs.
Who is eligible?
Individuals who are 60 years of age and older, reside
in the ten county service area, and meet financial and
need requirements. Eligibility is determined after an
assessment completed by an assessor from the Care
Coordination staff.
What services are
available through CARE?
Most of the services
listed are available in all counties, however, there
are some exceptions. Check with the Care Coordination
office for specifics.
What is the cost?
Care Coordination is
funded by a Senior Community Services Block Grant
allocated to the Area Agency on Aging District 7 from
the Ohio Department of Aging. Most eligible seniors
who are enrolled will find they have no charge for
their services. Those above the established financial
guidelines - based on the federal poverty guidelines -
may have the opportunity to share in the cost of their
services. The amount will be determined at the initial
assessment and agreed upon before services begin.
How can I make a
referral?
Anyone can refer a prospective client
for services by calling 1-800-582-9203. After the
referral information is taken, a face-to-face
assessment with the prospective client will be
scheduled for a later date. If a client is determined
to be ineligible for Care Coordination, other
referrals or traditional services could be discussed
with the individual.
